Ermac's "crunch you into a ball before slowly pulling you apart" thing. Kenshi's is similar but lacks the first part.

I'd say it's a tie between mileenas head snack, kenshi's pull apart, or uhm..yea Maybe bo rai cho's fart.

To me ALL of them look painful. Sure as hell wouldn't want ANYONE pullin a fatality on me.

I think Sub-Zero's second fatality would suck. Having your legs frozen then broken off while you're still alive would be insanely painful. Not to mention you then get your head blown off.
Dairou's is also painful. Imagine having two bones just torn out of your own legs and jammed into your eyes.

I think the one Tanya has looks pretty damn painful.. Would you really want a bone ripped from your leg, and the person waits just long enough that you can feel the pain from it before stabbing you through the throat with the bone?
Didn't think so.
Also, Kenshi's looks pretty painful... Being stretched apart is a pain I never have and never will want to feel.
Also, Scorpion's fatality where he rips your arm and leg off and then breaks your neck. That looks pretty painful.
Scorpion's head rip fatality might be a little painful. But you wouldn't feel anything for very long. All you'd feel is him getting the grip onto your throat, you choking, then off comes your head and thats it.

Bo Rai Cho's fart fatality would be pretty brutal... not only is his opponent burnt alive, but the last thing they smell (besides their own burning flesh) is Bo's beer-sour ass

Am I the only one who noticed that they stole Sub-Zero's trademark fatality, ripping off the head with the spine attached, and gave it to SCORPION of all people? I find something very wrong with that.
I don't really know about what the most painful might be, I couldn't pick just one. As for the worst fatalities, I'd have to go with the ones where someone's head is popped off, and that's it. Like Kobra's for example. BORING. We've only seen that done about a zillion times. However, this is why I love Li Mei's so much, because it tricks you into thinking it's just a normal boring decapitation fatality, then the head comes back down and BAM! She kicks it into the body, making it explode. Niiiice.

Haviks looks horribly painful too.
getting your leg ripped off and then beaten with it, and getting your arms eaten one little piece at a time really fast... ouch.

I thought the idea of two sais being kicked into my lungs an awfully painful fatality.

ok i will do this the math was.
i'll look at every fatality from the real point of view and then make a conclusion on which is the most painfull.
Sub-Zero:
1)freezes the oponent so that means the oponent doesn't feel a thing after he throws the head.
2)It's panfull after the leg destruction but after he steps on the oponent he emidetly dies and feels no pain
Scorpion:
1)when he tears out the arm and leg it's painfull but when he snaps the neck he's easing the pain and thats not painfull enough
2)The soconf one is painfull becouse it takes time for him to rip off the head
Mileena:
1) the sai thing is painfull becouse the just stay in your head and you feel the pain while you slowly die
2) The head-eater is very painfull becouse she chews alot on your neck and it's painfull
Baraka:
1) When he slices you it's not that painfull becouse his blades are very sharp and it's like when you cut your finger with a sharp knife you just see the blood and don't feel a lot of pain and after he rips the body out the oponent is dead.
2)it's panless becouse the oponent dies emidetly after the head rips off
Shujinko:
1) Where he slices teh head off is pailess becouse the oponent dies emidetly after the head rips off
2) When he slises up the oponent the oponent feels pain but for short time

1) When he ripps the arms off it's a lot off pain and then he hit's you with them that is even more pain
2)The picaso fatality is kinda painfull but after he removes the head the oponent is dead

1) When she screams it's very painfull to fall apart part by part
2) The helicopter thingy is not that paifull coz the oponent is dead after teh first hit

1) The leg rip is very painfull coz the oponent choaks to death and has a lot of pain becouse of teh bone in his neck
