Why these things still happen? (about glitches, edges that are turned a bit to quick...)

We've all noticed Rayden's cape in MKDA floating INSIDE of him.

Recently we've seen Baraka's hands going inside his blade.
And now... in Jades Fatality, her Green fabric piece that hang between her leg go through inside her as se pose...
Not to mention that it stays stiff when she bend.

Why do programmer don't notice and settle these things?

As a fan of wrestling games, I know these glitches are far too common. I don't think it's that they don't notice, I think they just feel there are more important things to worry about once you get to that point.
