this is a good question, since i unlocked the arena (very beautiful arena, i love it when it hails, the ghosts make the arena, even more beautiful) but i seriously do not know, i aswell would like to know aswell.

i guess its just like sarna ruins, what was sarna? maybe pekara was some sorta, warrior king who had a cathedral, i dont know, perhaps, since the house of pekara does seem to resemble a cathedral

Pekara could also be another realm that the house is located in. Or some other area of Outworld.

this is correct... it think... read it in the krypt... there are details about the places but it does not answer who pekara is though...
