If you look too, you can see that Smoke is transparent at certain parts, meaning that after all he is made up of smoke, just with the shoulder pads, mask, etc.

A blob of blood is obstructing his face. Looks like the mask is open-faced behind the blood.

Seeing as the blood actually appears to be gushing out of Smoke's face hole, instead of just being spawned at a random point, maybe this is a demonstration the Midway is stepping up the realism on the gore effects.

From what I can see, the only part of his face that is exposed is his eyes. But it is hard to say at this point.

eyes!!?? i dont even see any ..all i see is a white dot...like this ---> .

you have to zoom in... and its a whole head mask.. covers his whole face... and you can see his eyes.. but hmm looks transparent as well.. kinda... all well till we get a better pic at him.. thats when we can fully critic him... also i thought it was a hat at first.. but when i zoomed in the blood was abtracting my view of the whole mask..

just wait for a full body render of smoke to see what he really looks like, including his face/mask.
