What happened when Shang Tsung or whoever created her, they spliced Kitana's DNA into a tarkatan female. So she is half tarkatan and half human (well half kitana really)

maybe blades in the arms are exclusive to male tarkatan...

It would be pimp if they had a daughter, who would be Tarkata; Mileena would move up on the scale and become a sub-boss/boss, and the daughter, in her thirst for gore and glory, would murder her own mother in her own attempt to be queenie.

There have not been any true female Tarkatans in the game yet. In Baraka's ending it is said about Mileena that her sense of smell was not that of a "true Tarkatan." She is mainly Kitana DNA, with a little Tarkatan thrown in around the mouth area; hence no blades. Tarkatans are Netherrealm, Outworld hybrids; there must be a few female Tarkatans out there, after all they are their own race now. Like how cockapoos used to be half cockerspaniel and half poodle, now they're their own breed. There are female Tarkatans, just none are in the MK series.

Well, we dont know for sure. Mileena probably doesnt resemble female tarkata in any way.
I reckon that the tarkata all look the same. male and female only differ slightly and probably only they can tell by the smell.
