That stage doesn't have a stage fatality (noted by the absence of a skull icon if you select it manually). As for the fire, beats me.

Its yet another one of those stages that was supposed to have a Death Trap, which was scrapped last minute, sort of like the Beetle Lair and the Chamber of Artifacts. I was a little disappointed that the dragon head doesn't spit fire, as its picture clearly indicates.
