Here's the secrets I've found, as posted in my secrets thread:
Goro's Lair
First Room: Throw an enemy into the skeleton on the wall.
Second Room: Above the entrance. Do an air combo or come back when you get wall run.
The Pit: Shoot the moon until it appears. Also, drop off the left side onto the ledge.
Fatality room: Go through the Ko-op room to get to it. Also uppercut an enemy into the chandalier.
Fire/Cave Room: Knock four enemies into the spikes. It will appear on the left side of the room.
Oni Warlord Room: Drop off hte left side of The Pit onto the ledge.
Wu Shi Academy
First Area: Knock an enemy into the third catapult to reveal a secret. Also, in Ko-op a secret appears near one of the barricades.
Second Area: Ko-op switch. In the Ko-op room there are two secrets. One of them requires the double jump.
Area with portal: Use the double jump to get the secret above the spikes. You can use enemy bodies to get up there, but it takes longer. Also, use the swing to get across the gap near the portal.
Burning area: Under the final test your might. Jump onto the house near the first test your might, then jump into the hole.
Warrior Shrine: Beat Ermac.
Portal
Ko-op switch.
Do a fatality on the dragon logo near the entrance to the Foundry.
Evil Monastery
First Room:Break the window with the dragon logo behind it. Throw a projectile at the dragon logo.
Second room: Ko-op switch. Requires Fist of Ruin to get the secret.
Third Room:Knock enemies onto each of the statues claws. The secret will appear behind the statue's head.
Multality Room: Jump on the half broken pillar to the broken ceiling.
Boss Room: Return to this room to find a secret.
Living Forest
First Area: Swing on a branch near the save point to get to the treetops. There is a dragon logo to shoot and a fight with Mileena that awards you with a secret.
Second area: Throw a clay soldier into the wall behind them.
Third Area: Throw a clay soldier into the wall behind them to find Smoke. Throw one into the pillar on the right side, then return to the last room to find a secret where the waterfall was. Also, there is a secret behind the living tree. Requires double jump to get.
Fourth Area: Jump over the wall to the left of the snake head. Also a health upgrade to the right of it that requires wall climb.
Brutality room: After beating Reptile, return here and break more statues. Also a Ko-op switch that I haven't activated yet.
"Hall of Eyes": Shoot the eye that is closed.
Soul Tombs
Entrance: Pan the camera to the left to see a dragon logo. Shoot it.
Main Area: Above the door to Baraka. Wall run then double jump. Also obtainable with an air combo.
Through the red door: Throw an enemy into each of the spike traps.
Room before Hellbeast: There is a Bloody Skeleton in the background. Shoot it or throw a skull at it until it dies.
Hellbeast Room: After beating Hellbeast, return then stand on the block until it sinks into the ground.
Other room after the red door: Break the statue here to reveal a secret. Then go past where the statue was and jump to another secret.
Final room after red door: Throw an opponent through the wall to reveal a dragon logo. Shoot it.
Room where you cut the bodies: On one of the chains moving up and down is a set of spikes. Throw an enemy onto them.
Shang's Soul Vessel: Return here for a secret.
Platform section: Uppercut an enemy into a pterodactyl on the middle platform.
Fan room: Wall run up a pillar or long jump off the fan.
Staircase to fan: Break the statue.
Wastelands:
First room: Uppercut an enemy onto the spike in the center. Also one in an alcove above the save point.
Kabal room: In plain sight.
Elemental Knight room: Return here to fight an Oni Warlord. Beat him and the coin appears.
Castle entrance: On the left lion. Wall run/double jump to get it.
Wheel room: Wall run above the wheel to get to the other side.
Room before Goro: Pan the camera to the right. Shoot the gold coin you see.
Goro room: Return for a secret.
Dead Pool: Above one of the grates in the back of the room. There's a certain requirement to get it, but I'm not sure what it is.
Netherrealm:
In plain sight above an arch. Double/long jump to it.
Foundry coming soon.

OK I give up. how do you go back to goro's lair once you get the wall run? I can't find a portal or a way back walking.

I am new to this community. Like many here I have been a long time MK fan. This site has helped me with the last few MK games and I found a couple of things in MKSM that I haven't seen posted so I thought I would share.
On the way to the Hell beast in the Soul Tombs there are a couple of rooms. In one of them you can see a bloody skeleton walking in the background room. Lock on and fire your projectile, kill him and a coin appears in the room you are in.
In the other room, I believe the one where the Tarkatans molest you; you can find a coin in the left hand corner closest to you up high, I wasn't able to see it but I came across it by using Kung Lao's repeated air strikes.

Here are a few updates/errors for your guide!
1. Sub-Zero's icon location is in the Wastelands.
2. You do not need to get the double jump ability in order to reach Sub-Zero's versus mode icon. Just do an angled wall run onto the statue.
3. In order to get to the Ermac boss battle, you need the Fist of ruin and the swing ability. Not the Fist of ruin the wall jump ability.
4. There is a space inbetween "f" and "ew" in the Ermac paragraph.

i have 67 of the 68 total secrets at this time and i am at a 102.4% complete. and i am missing johnny cages head decap fatality input image for the koncept art! can someone please tell me where to find that secret cause this is the only one that i am missing!

You unlock Mortal Kombat II after completing all five of the Smoke missions. Look at the first page of this topic or at mkkools website for more information on completing the Smoke missions!

I think you get the Warrior Shrine by activating the switches in ko-op by the armory where you team up with Johnny Cage.

in the wu shi level where you first see the catapults go up the stairs (where you have to kick the enemy in the pile of spiked rocks), to the left of the stairs there is a roadblock which can be destroyed by using one of the catapults (but dont remember which one). there you'll find the warrior shrine vs level orb. the funny thing is that i'm nearly 100% sure that this orb wasn't there in solo play and only in ko-op. can anyone confirm this?

I remember hearing this as well. I think it's also mentioned in the Stratagy guide as well.
