Most people most likely have got these, but I thought I would put them down anyway. The locations are correct, but finding some needs some looking work.
Kira is found at H-2 at night in a house around there. Earthrealm.
Lui Kang's Tomb Stage is found after fighting Jax near a bridge. Cross the bridge and you'll find a treasure box there. It has to be at night. The location is H-6. Earthrealm.
The Nexus Stage is in the Chaosrealm. A-4. Early Morning is only when you can get this treasure box. It is near where you get the portal key for this world's quest.
Tayna is located inside an ice cave in Outworld. A-3. You have to wait until night time for the treasure box to appear. The ice cave is not hidden that well. It is also part of a side quest that Frost sends you on.
Hotaru is in the Orderrealm. H-1. This treasure box appears at early morning.
Quan Chi's Lair is in the Orderrealm. H-5. It is inside a house. Treasure box.
Kutan Palace is in Orderrealm. A-4. Inside a house. Treasure box.
Golden Desert is at E-2 in the Netherrealm. Hidden well. Treasure box.
Living Forest is unlocked in the Krypt. DS are the letters.
The Portal Stage is in Earthrealm at G-5. Well hidden area. Treasure box.
Dead Pool stage is unlocked in the Krypt at letters JI.
Kenshi is in Earthrealm at C-3. Not that hard to miss the treasure box.
Havik is in Chaosrealm in a house at H-4. Treasure box.
The Courtyard stage is in the Netherrealm at A-1. Not hard to miss in a treasure box.
Here are a list of non-death trap stages:
The Bettle Lair- has a weapon on the second floor.
The Courtyard
Liu Kang's Tomb- has a weapon.
Living Forest
Portal
Netherbelly Boat
The Glass Tower- has a weapon. No matter how many times you break the glass, nobody goes through it.
Who is suprised by these not being a death trap as a question?

Here are what some of the krypt letters contain in case you were wondering:
QT- Hotrau's Alt outfit.
QP- Ermac's Alt outfit.
OI- Jade
TB- Darrius Alt outfit.
SC- Kenshi Alt outfit
SG- Kira Alt outfit
GO- Jade puzzle kombat character
IB- Bo Rai Cho's Alt outfit
MI- Kabal puzzle character

the beatle liar doesn't have a death trap... it has only weapon, it hows it in the level select.
Nexux has a death trap... even if it looks like it doesn't... that is cool

I have unlocked every character except Lui Kang and this "Mystery" character.
Dragon King Temple is only available after you beat Konquest mode. Once this task is done, go to the Chaosrealm and use the teleports that didn't work before. These take you to different places. D-7 is where the Dragon King Lair is located inside a house. I got it at night, but I am not sure time matters. In these areas you learn Kobra and Noob Smoke. You have to find them. Noob is in what looks like a prison. Kobra is in the area with many temples. Most of the houses and temples have Koins to collect.
Raiden I unlocked after I beat Konquest mode. Same area as before, E-3. He stands in the same spot with his guards. Defeat him to get his key.
Earthrealm has one area that is not accessable until Konquest mode is beat. It is the area past the bridge you fight Jax on and found Lui Kang's Tomb. H-6. Pass that to the left. There you will learn Lui Kang's moves as he "teaches" you.
PP in the Krypt is Raiden's puzzle character. Not worth the money that it wants.
Li Mei is only after you are captured by Kano and brought into the village. She is located in a house at F-7. The same goes for the Dragon Mountain Stage, It is at H-8. This event is far into Konquest mode.

DO NOT READ IF YOU DON'T WANT TO KNOW THE WHOLE KONQUEST STORY!
Ok, so you are the champ elder god, Shijuko. You have been chosen to go on a lifelong quest to find these missing artifacts. A clould puff named Damashi is your "guide" throughout your adventure. First off, your character would have entered the original MK tournement, but Damashi thawrts that possible outcome. A few times along your adventure, you try to help people, but it seems that Damahi doesn't care for these events. Seeing a trend here...
Along the way you get your soul cleaned up by Nightwolf, but then you have to go to the Netherrealm to unclean it by order of Damashi. Also you learn a whole lot of characters and fight alot.
After you are semi-old, you are arrested by Hotrau and put in jail until you become a grandfather. While in the prison, Dariou busts you out and tells you that he was sent by someone. He was paid for it.
To finish up this tale of betral, you find out that the Dragon King was Damahi all along. He decieved you into getting him all the artifacts so he could rule all the realms. The final end to this tale is to beat the Dragon King in the Arcade mode.
There is your Deception.

Shujenko's ending: DO NOT READ IF YOU DON"T WANT TO KNOW!
Have been led through deception in his lifelong quest, he gained the powers of many warriors. Through this power, he used it against the Dragon King and brought the realms back to what they were at the beginining. (Cool shot of all the warriors blasting the Dragon King with there powers.)
