Currently? Shao Kahn.

If not: Onaga or Mileena?

Probably Mileena.

The Elder Gods practically erased Shao Kahn from existance, Shao Kahn transferred Shang Tsung's power and lifeforce into Sindel and Nightwolf performed an ultimate sacrifice on Sindel. Sindel was seen in the Netherrealm but not Shang Tsung.

So I'm sure Mileena is the answer...........if her electrocution at the hands of Raiden didn't kill her. (After defeating Mileena during Stryker's chapter)

Mileena wasn't killed by that blast, in a cutscene shortly after that, she returns to Outworld to bring Shao Kahn news that, "Raiden has killed Motaro", she's a survivor.

Okay, since Big_Speck hasn't given any life signal(on the forums) that ^ is the current question...

I think it's Mileena...But everyon has said this, so the next ruler will be...Shinnok?

I think the most likely one is Mileena, as Shao Kahn calls Mileena his "daughter", after imprisoning Kitana at the end of her chapter. The supposed offspring of the previous ruler must be the heir to the throne, right?

So now the Ruler of Outworld is Mileena.
