At one point, the story was that he was killed by Kintaro in MKII.

But then they changed it so that he was killed by Shao Kahn's Extermination Squads (lead by Shao Kahn).

Then with MKDA, it was changed so that he wasn't killed at all.

Well it was Kahn's extermination Squads lead by Motaro that killed Johnny Cage.

But, the opened portal in the Earthrealm blocks Johnny Cage soul, to go to the Heaven. So he decided back to his body and fight alongside his friends. After the battle, cage ́s soul go to the Heaven.

And there he witness the battle of Quanchi against the Gods. he asks Raiden to ressurrect him and it is done. Johnny is back.

Cage did die, he came back to life in his MK Trilogy Story.

I believe it was John Vogel that said that he never died. It does say that he was killed in Trilogy but he decided to say that he never died now.

How Cage Died: After Kintaro kicked his ass, then Kintaro ate Cage.

I think of it like this, when Shinnok had returned from the Netherealm thanks to Quan Chi, he had a vendetta against the elder gods who had banished to the Netherealm in the first place. Shinnok invaded the heavens with a legion of demons and wiped out almost all of the gods exept for Fujin and Raiden.

It was a desperate time for the Elder Gods and Johnny Cage was one of the survivors in the heavens, so I believe that the only reason why Cage was allowed to return to life was because the Elder Gods had no other choice. The balance of the realms were twisted now that Shinnok had creatures from different realms roaming other realms. Edenia was taken over by Shinnok and Earthrealm was pretty much everyone's last hope in defense against the fallen Elder God. So Johnny Cage who fought alongside Liu Kang and the others was reluctant to join and help fight against this evil and the Elder Gods saw his cause. I think that also Johnny Cage fought so that mabey after Shinnok was destroyed he could have a second chance at life and he could have struck a deal with the Elder Gods to fight for them.

However, that whole storyline was changed due to John Vogel's new retcon to his damn story. A shame...
