3*Now You'll see the Ultimate Kombat Kode, What is that for?
The Keymapping is as Follows:
1: HP (B).
2: Blk (C Left).
3: HK (C Up).
4: LP (A).
5: Run (C Down).
6: LK (C Right).
and...
L: 4th Slot.
R: 1st Slot.

I dont think it was ever found. I think the only way to input the code is via push button code on the intro screen.

That UKK screen was never completed before the game's launch. There's no codes implemented into it.

I am 99% sure that the code simply is an alternate method to obtain Smoke in his human form. If you get a version 2 Gameshark for N64(the one with the hacking abilities), you can view just about all of the text in the game. I did a search for the Ultimate Kombat Kode and when I found the congratulatory message, it mentioned Smoke was unlockable. Still, the message for a correct code entry was probably put in before the code was implemented or, like everyone said, it would have been found already.
What was really interesting was that I searched more of the text and found not only the names of Goro and Kintaro (who are missing on the N64), but also a text saying, "Boon Wins". I really doubt Ed Boon is a secret character, but I thought it was funny nonetheless.

It must unlock both Khameleon and Human Smoke. If you use the cheat menu to unlock one then the UKK is still there, but if you use it to unlock both it is disabled.
One of the "secrets of MKT" in Kahn's treasures gives you the UKK but it doesn't work. I'm guessing they decided to change it late in development and disabled it yet never got around to adding a new one.

I got GameShark Pro 3.3, it has a Parallel Port at The Back.
I got The Driver and Stuff, but if I Connect it to The PC, MKT gets Frozen.

I remember that at a Time, I attempted to Overwrite Character Names.
