You may have to be 2 steps away from your opponent for it to come out (same as the fatality distance). But I'm not sure, it might just work anywere.

In the early version of MKII you could do Sub's Fatality freeze in the middle of the game, and it would leave them frozen the whole round.

for real? even if we hit the frozen guy during the round, he will stay frozen?

I mean he just stays frozen, you hit the fatality freeze and they stay like that the rest of the match, if you hit them they unfreeze.

You can be any distance, but you have to be on the left side.
It's probably the most broken thing in the series' history.
