UMK3 says Shao Kahn brought her back to life but that was retconned in MKT. Shinnok brought her back to life.

We don't know if her falling in the trap door thing in MKG was canon. Just because some endings were used doesn't mean that one was. They could simply ignore it when she returns in MKD.

At this point the only time she died was when Kitana killed her during/after MK II.

the thing I don't understand about these games, is which endings actually happened? I mean, Liu Kang won the first 2 right? doesnt that mean that everyone else's endings never happened?

How do we know the continuing story up to this point?

She died in MK2 (then Shang ans Khan revived her... it's in her story -not ending-) and in MKT/MK3U because Shinnok then revived her in MK4G in the Netherrealm (again in her story, not ending)

SHE HAS DIED 2 TIMES:

She was killed in Mk2 by Kitana, and revived by Kahn in Mk3.

She was killed in Mk3 by Kitana, and revived by Shinnok in MkGold.

She probably NEVER died in MkGold since we see her alive for MKD.

Where does it say she died in MK3? In MKG it says she was resurrected by Shinnok after her death, but its doesnt say she was killed in MK3.
In Mileena's MKT ending it says (I'm paraphrasing) "It was really the fallen Elder God Shinnok who resurrected Mileena after her death at the hands of Kitana. He sent her back to Earht to spy on Shao Kahn..."

She has died ONCE.

Shinnok revived her that one and only time.

Her MKT ending says it was Shinnok, that overrides what they say in her opening bio or what it says in UMK3.

Directly from MKT...

"When Mileena is murdered, she finds her soul is reborn in an evil place called the Netherealm. She accepts servitude to an all mighty god
known as Shinnok. It is Shinnok who allows her soul to be reborn."

Kitana killed her in MK II. Shinnok allowed her to return. In MK3 she went back to working with Kahn, while secretly being with Shinnok and planning to bring Kahn down. After Kahn's fall she left and sided with Shinnok for MK4.

When I said Mk3, I was referring to MKU and MKT, remember those are updates of Mk3.

Weird, I dont remember ever that MKt ending. Its very strange cause her Bio in MkU/MkT clearly say she was revived by Kahn.

What if she died and was brought back to life (like current mortals do in real life; hence mortals) and the Mortal Kombat producers didn't catch it because they were off filming some other character? ;) Haw. Never thought about dying and somehow coming back to life behind the producers backs now DID you?
