I think Mileena has always been a fan favorite, but her popularity really skyrocketed after Deception. It wasn't just the heavy promotion she recieved, it was her presentation: The look, the story, the gameplay, and personality.
She was just very well done in MKD. Slap that all over covers, websites, etc. and you've got an instant series icon.
So in conclusion, she's popular because she's high quality in all aspects and heavily promoted.
That's the recipe for a huge fanbase.

I have always been a huge fan of Mileena, I remember when Mortal Kombat: Annihilation came out, I was really upset with the lack of importance/storyline she had in the movie because she does play a pretty important role in the series. Once Deception came out, she was used as one of the main market tools..Her render was in every game website, the video game stores had Mileena stands and posters and as a result, her popularity increased and people became interested in her. After all, she does have that "looK" that catches peoples attention.
In my opinion, I really like Mileena because of her storyline. She was MK's DEBUT female villain and I like the fact that she's a villain who is full of anger, visciousness, vengeance and hate towards others, but can also be very loyal to her masters and allies. Her storyline is much deeper than any other female in the series (Shao Kahn favored Kitana over her, she's only a "clone," and she constantly has to prove herself to others. She doesn't really rely on "special powers," she just goes in and kicks butt. I really think that Mileena is Mortal Kombat's "MAIN" female villain. She's like the Catwoman, Bellatrix, or Mystique of Mortal Kombat.
